Vision Insurance: Eye Med

We went live with vision benefits through EyeMed as of January 1, 2025. This brought us an increased benefit at a lower price and has afforded more local locations that accept our coverage.


As a reminder, plan participants and their covered family members do not need to have an ID card to utilize vision benefits.


When attending an appointment, plan participants can supply their name and DOB and the provider should be able to find them in the EyeMed system.


If you did not receive an ID card and would like one, you can register and login to the EyeMed portal and find the card under Account>Print My ID Card. You may also request a card by calling EyeMed Customer Service at 866-939-3633.


If you believe you have eye insurance coverage but your provider is unable to locate you in the system, please contact Shanell Lemons, Human Resources & Training Manager at 715-768-9013.
